Monarch, Bristol Park, and MHAP Contracted Provider
Dr. Allen Doezie is contracted with Monarch Medical Group HMO insurance for all breast reconstruction cases, Bristol Park HMO and MHAP insurance for any case that is medically necessary (non-cosmetic). Patients simply need to call their primary care physicians (PCP) and request for an authorization to come to our office. Having an authorization as an HMO patient means our office will collect an office co pay and the remainder of the office visit will be billed to the insurance. Patients can call our office and ask us to check the status of any requested authorization.
